Every pet needs a Check-In Form for every visit. Print it ahead of time or fill it out at drop-off.

Call or send a request
Tell us about your pet and the dates you have in mind. New dogs start with a free trial day, which we set up by phone.
Send vaccination records
E-mail, fax or bring in proof of current vaccines. No pet can be admitted without them on file.
Free 4-hour trial day
New dogs spend a free 4-hour daycare session with us, Monday to Thursday, dropped off between 10am and 12pm, so we can get to know them.
Forms and drop-off
Fill out the Pet Agreement once and a Check-In Form every visit, then bring your pet, their food and any medications.

Questions About Forms
Do I need to fill out the forms every time?
The Check-In Form, yes, every visit. The Pet Agreement and the Veterinary Treatment Authorization Form only once, unless our policies change.
Can I fill out the forms at drop-off?
Yes. Printing them ahead of time just saves a few minutes.
My pet is diabetic. Which form do I need?
The Diabetic Form, in addition to the Check-In Form and the Pet Agreement.
